The Centers for Medicare and Medicaid Services (CMS) is responding to the lack of Medicare-funded technology support for patients managing chronic conditions by introducing the new Advancing Chronic Care with Effective Scalable Solutions (ACCESS) Model. ACCESS will provide new payment options for clinicians to offer technology-supported care, including remote monitoring devices and wearables for chronic conditions, including early cardio-kidney-metabolic, cardio-kidney-metabolic, musculoskeletal, and behavioral health, with additional tracks and conditions to be considered for future model years.
